DA & AR Hospice Care, Inc. was founded and originally licensed by CA Department of Public Health in 2014. DA & AR Hospice Care, Inc. had a change of ownership completed on February 2018. Shortly after the change in management, DA & AR Hospice Care, Inc. received a deemed status by CMS through the full accreditation of The Joint Commission on June 2018.

The new owners and management team of DA & AR Hospice Care, Inc. have proven their compliance, skills and knowledge with the coordinated patient-centered health care delivery system. The evidence of this statement was validated by the completion of 3 full days of unannounced onsite survey conducted by The Joint Commission.
